CLAIM PROCESS

Submit a police abstract immediately when report the incident/claim

Submit the duly completed claim form together with supporting documents

In case of death, the next of KIN to submit the death certificate and burial permit

Where there is no beneficiary nominated, the dependant of the deseased or the adminstrator of the deseased's estate should provide proof of the dependancy or letter of adminstration

FOR GROUP POLICY CLAIM

If you have a Group Personal Accident Policy at work, your employer will be responsible to submit the claim to the company.  The claim will be payable to your employer and the actual disbursement of the claim amount to you or your family at the discretion of your employer
